Note: This position is for PRN, hours are 8:30am-5:00pm Weekdays.
Responsibilities and Essential Job Functions
Demonstrates appropriate assessment and intervention skills in meeting the psychosocial needs of Home Health and Hospice clients and families.Documents all assessments, interventions, and contacts.Demonstrates awareness and competence in working with clients and families within the context of disability, age, ethnicity, and culture.Performs bereavement assessment and counseling per protocol to hospice families/S.O. after patient death.Offers bereavement counseling to community members on an as needed basis.Facilitates bereavement groups as scheduledAttends and participates in IDT meetings.Communicates appropriately and effectively with clinical and office staff in meeting client needs.Identifies self and other staff learning needs and participates in professional growth, i.e. teaching, education and professional associations.Demonstrates the knowledge and ability to utilize community resources to meet the needs of the Hospice and Home Health patients and families.Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.These statements are intended to describe the essential functions of the job and are not intended to be an exhaustive list of all responsibilities.
