Job Summary and Responsibilities
Hospice Social Worker
Join our multidisciplinary team as a Hospice Social Worker! You'll be responsible for assessing and supporting the social and emotional needs of patients and their families, promoting continuity of care and services. This vital role involves psycho-social assessment, mental health evaluation and referral, crisis intervention, environmental assistance, patient advocacy, and after-care planning. Assignments may include coverage at multiple locations and settings, with consistent job requirements across all.
Job Requirements
Education & Work Experience requirements:
Master’s Degree in Social Work required
Two years recent related work experience that would demonstrate attainment of the requisite job knowledge skills/abilities is preferred.
Licensure & Certification:
Current Licensed Independent Clinical Social Worker (LICSW)
Current Licensed Advanced Social Worker (LASW)
Or
Eligible for Agency Affiliated Counselor; and the state must receive the application within seven (7) days of employment; and the credential must be obtained within 60 days of hire. Licensure must be obtained within 42 months.
