clinical services across the continuum of care Maintains ethical principles and professional standards of practice in adherence to the disciplinersquos professional code of ethics Assumes leadership responsibilities as assigned by clinical leadership May assume clinical oversight of daily operations in the absence of manager Setting of clinical care and population served will depend on unique needs of Business UnitJob responsibilitiesConducts a comprehensive bio psychosocial assessment and establishes a care plan as appropriateCollaborates with the interdisciplinary Team to assess for appropriate level of care and risk factors develops and implements care transitiondischarge plan and coordinates consultsreferrals to remove barriers and promote optimal clinical and financial outcomesUtilizes specific therapeutic methodsstrategiesinterventions in accordance with individualized care planMonitors progress of interventions and revises plan to meet patientfamily needsProvides education to patientfamily with respect to meeting established goalsProvides direct patient care that may include case management care coordination interdisciplinary collaboration for care transition needs crisis interventionsafety planning counseling and emotional support to patients and families coping with end of life careRefers patient to appropriate levels of care and available resources that promote the individualized care planMaintains timely patient centered documentation that reflect the patient and client systemsrsquo pertinent information for assessment interventions and outcomes in accordance with regulatory licensing requirements and third party agenciesIndependently able to provide clinical social work interventions to all patient populationsAssists in the coordination of
