Hospice RN Case Manager Location: Coverage area spans Brookline/Brighton to Mansfield (scheduler will cluster visits by area) Schedule: Monday–Friday | 9:00 AM–5:00 PM (holidays off) Start Date: 7/6 for contract; ASAP for direct hire Assignment: 26 weeks to start (Temp-to-Perm & Direct Hire available) Compensation + Contract: $100 per visit + Direct Hire: $110,000–$120,000/year (commensurate with experience) + Mileage Reimbursement: $0.725 per mile + Parking: Fully reimbursed About the Partner Organization Amergis Healthcare Staffing is partnered with a multicultural boutique hospice agency delivering compassionate, patient-centered care across homes, nursing facilities, and assisted living communities. Their diverse and multilingual team is dedicated to honoring cultural backgrounds, engaging families, and supporting dignity during end-of-life care. As a Hospice RN Case Manager, you will play a vital role in guiding patients and families through the end-of-life journey, focusing on comfort, symptom relief, and holistic support. Key Responsibilities + Conduct 5–6 home or facility visits per day to assess patient condition + Collaborate with physicians and interdisciplinary team members on individualized care plans + Provide comfort-focused care based on each patient’s unique needs + Partner with caregivers to ensure continuity and quality of care + Administer medications and manage pain/symptom control + Respond to acute changes in patient condition and escalate to physicians as needed + Document all care thoroughly and accurately + Maintain high ethical standards and adhere to safety protocols + Demonstrate sound clinical judgment, attention to detail, and compassionate communication Working Conditions + Care delivered in patient homes, assisted living facilities, and nursing homes + Ability to travel throughout the designated service area + Remote work as needed for documentation + Valid driver's license required Member-Facing: Yes — this position requires in-person patient interaction Requirements + BSN with an active, unrestricted RN license in Massachusetts + Minimum 1 year of experience in hospice, palliative care, home health, or long-term care Preferred Qualifications + Experience working with electronic medical records (EMR) + Background in care management Direct Hire Benefits + 401(k) with 4% company match + Health and dental insurance + Flexible spending account + Mileage reimbursement + 3 weeks PTO to start + Annual bonuses + Free lunches + 15 paid holidays Interested in Learning More?
