AidesTypical Physical Demands Requires full range of body motion including handling and lifting patients manual and finger dexterity and eye hand coordination Requires standing walking and the ability Instruction of patientfamily in procedures or responsibilities they are assuming and optimum wellness practicesh Planning for the patients discharge from servicei Appropriate implementation and utilization of OASIS systemj Consults with patientfamily regarding the process of dealing with issues of ethical concern Documents all services provided in the clinical record in accordance with state and federal regulations and Agency policya All records are complete accurate and done in timely fashionmiddot admissions within hours from first visitmiddot subsequent visits within hoursb Obtains physician verbalwritten orders and complies with all reporting requirementsc Participates in coordination of care with physicians and with other disciplines as appropriate Manages work assignment in effective and efficient mannera Reports any cases to the HospicePalliative Clinical Supervisor where goals can not be met or at risk situations exist and responds appropriatelyb Remains flexible in daily planning allowing time for emergencies or unexpected visitsc Maintains productivity as established by the Agencyd Utilizes payor sources correctly reflecting changing levels of caree Delegates patient care responsibilities to Home Care Nurse LPN as appropriatef Participates in the development of the hospice interdisciplinary team care plan Establishes the plan of care to be delegated to the HomemakerHome Care AideCompanion orients and supervises the home care aide in accordance with state and federal regulations and Agency policya Develops an initial written plan of care for the HMKHCACompanion and updates this plan as patients condition warrants but no less often than every daysb Obtains input from HMKHCACompanion regarding patient progressc Prepares written performance evaluations of home care aides under hisher supervisiond Provides supervision of home care aides at a minimum of every two weeks or as required by payor source Develops and maintains sufficient professional skills and credentials to meet the needs of patient carea Attends a minimum of hours of in service per year At least six of those hours shall deal with aspects of hospice careb Participates actively in self appraisal and the formation of a plan of self developmentc Knowledgeable of agency policies and procedures Adheres to Agency Corporate Compliance Plan and HIPAA Privacy Plan as per written agreementd Seeks appropriate clinical input to ensure appropriate patient caree Projects professional image through attitude and compliance with dress codef Capable of precepting new nurses and studentsg Functions as a team member with consideration to all staffh Provides Agency with current copy of professional licensei Maintains CPR certificationAncillary Duties Performs Skill in applying and modifying the principles methods and techniques of professional nursing to provide ongoing patient care Skill in identifying patient related problems and recommending solutions Skill in documentation in clinical records Skill in establishing and maintaining effective working relationships with patients families physicians co workers and the community Ability to maintain quality control standards Ability to react calmly and effectively in emergency situations Ability to interpret adapt and apply guidelines and procedures Ability to communicate clearly Possess adequate
