HomemakerHome Care AideCompanion orients and supervises the home care aide in accordance with state and federal regulations and Agency policya Develops an initial written plan of care for the HMKHCACompanion and updates this plan as patients condition warrants but no less often than every daysb Obtains input from HMKHCACompanion regarding patient progressc Prepares written performance evaluations of home care aides under hisher supervisiond Provides supervision of home care aides at a minimum of every two weeks or as required by payor source Develops and maintains sufficient professional skills and credentials to meet the needs of patient carea Attends a minimum of hours of in service per year At least six of those hours shall deal with aspects of hospice careb
