Job Description:

Hospice RN Case Manager
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Job Type: Full-Time | Field-Based
Territory: Battle Creek → Marshall → BellevueBring Care, Comfort, and Dignity Home

What You’ll Do

Deliver compassionate, high-quality nursing care to patients in their homes

Conduct comprehensive assessments and create personalized care plans

Anticipate and respond to changes in patient conditions with clinical expertise

Partner with physicians and the interdisciplinary team to coordinate care

Educate and support families through the emotional journey of end-of-life care

Ensure care is delivered in alignment with physician orders and regulatory standards

What You’ll Bring

Graduate of an accredited nursing program (Diploma, ASN, or BSN)

Active RN license in the State of Michigan

At least 1 year of nursing experience (hospice or home health strongly preferred)

Reliable transportation to travel within your territory

Strong clinical judgment, communication, and organizational skills

Physical ability to meet the demands of home visits, including lifting up to 50 lbs
