What You’ll Do as a Hospice RN Case ManagerConduct patient assessments to identify physical, psychosocial, and environmental needs
Develop and implement individualized care plans with the interdisciplinary team
Monitor changes in patient conditions and communicate updates to the care team, physician, and family
Educate and support patients, families, and caregivers in following the hospice plan of care
Provide teaching on hospice philosophy, pain control, and symptom management in various care settings (home, nursing facilities, assisted living, etc.)
Offer compassionate support at end-of-life and assist with bereavement needs
What We’re Looking ForGraduate of an accredited nursing program (Diploma, Associate, or Bachelor’s)
Current Registered Nurse (RN) license in the state of practice
1+ year of experience in a clinical care setting (hospice or home health preferred)
Ability to travel (50% of the time) with reliable transportation, valid driver’s license, and auto insurance
Physical ability to sit, stand, bend, lift, and move intermittently (up to 50–100 lbs)
