As a Registered Nurse Case Manager, you’ll be the heart of hospice care—guiding patients and families with compassion, skill, and personalized support. You’ll manage a thoughtfully balanced caseload (typically between 12–15 patients) that allows you to provide meaningful, high-quality care without feeling overwhelmed. Best of all? No on-call requirement, so you can focus fully during your scheduled hours—and truly unplug when you’re done.
Schedule: Monday through Friday 8:00a to 5:00p

What You’ll Do:Assess physical, psychosocial, and environmental needs of hospice patients and their families
Develop and follow individualized plans of care in collaboration with the interdisciplinary team
Educate patients, caregivers, and facility staff on hospice philosophy and symptom management
Communicate significant health changes to supervisors, physicians, and care partners
Provide emotional and spiritual support at end-of-life, including post-death bereavement assessments
Ensure compliance with Elara Caring’s mission, values, and hospice best practices
What You’ll Need:Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Bachelor’s)
Current RN license in the state of practice
Minimum 1 year of clinical experience (hospice or home health preferred)
Valid driver’s license, car insurance, and reliable transportation
Able to lift/move 50–100 lbs and travel locally (approx. 50%)
