**Full time Hospice Liaison RN -Albany**
The Community Hospice has a rewarding opportunity for an RN to work as a part of our Hospice Team serving as Liaison in local Nursing Homes .
HOURS : 8am-4:30pm (can be flexible).

**What you will do:**
+ The Liaison RN evaluates patients for eligibility and interest in hospice services, describes the support and care available from hospice along with the benefits of hospice care, and coordinates with the patient’s community physician, facility, and our hospice medical directors to confirm eligibility for hospice services.
+ Admits patients to hospice in the nursing home or ensures a smooth handoff to the hospice RN admitting the patient/family to services.
+ The work schedule is flexible, generally M-F weekdays 8am to 4:30pm .
**What you will need:**
+ NYS RN Licensure
+ ASN required, BSN preferred
+ Requires at least 1 year experience as an RN in an acute care setting
+ Homecare experience required
+ Hospice experience preferred
+ Reliable vehicle and driver license
+ Excellent communication skills. Must be able to communicate effectively with patients, physicians, and peers.
+ Must be able to document key information concisely and accurately
+ Strong computer skills
+ Strong organizational and time management skills
_Pay Range: $35.00-$47.75_
_Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location._
