Are you a **Registered Nurse** with Skilled Nursing Facility (SNF) experience who thrives on building relationships and making a difference in the lives of seniors?

We’re currently seeking a compassionate, dependable RN to serve patients in **Skilled Nursing Facilities throughout the Dallas area** —while also serving as a **liaison between Gentiva and facility staff** to ensure quality, coordinated care.
This unique role is ideal for an RN who enjoys hands-on care **and** the opportunity to collaborate with facility teams, advocate for patients, and represent hospice services in a professional, positive way.
**What You’ll Do:**
+ Deliver compassionate, high-quality hospice nursing care to patients in SNFs and ALFs.
+ Collaborate with facility staff and leadership to ensure seamless coordination of care.
+ Act as a liaison between Gentiva Hospice and partner facilities, fostering strong, positive relationships.
+ Provide education to facility staff on hospice services, philosophy, and best practices.
+ Advocate for patients and families while supporting facility teams in meeting their care goals.
+ Document patient care, updates, and facility interactions accurately and timely.
**About You**
**Qualifications – What You’ll Bring:**
+ Current RN license in good standing.
+ Experience in hospice, palliative care, or skilled nursing.
+ Strong communication and relationship-building skills.
+ Compassionate, patient-centered approach to care.
+ Ability to collaborate with facility leadership and staff while maintaining professionalism and empathy.
+ Valid driver’s license, auto insurance, and reliable transportation.
+ CPR certification required.
**Preferred Experience (Not Required):**
+ Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac)
+ Admissions RN, wound care, float pool, travel nursing
+ Experience with terminally ill patients or serious illness support
