development of individualized care plans middot Certify re certify when necessary patients eligible for hospice care under the Medicare Hospice Benefit middot Consult with attending physicians regarding pain and symptom managementmiddot Coordinate efforts with the attending physician to provide continuity of caremiddot Develop and monitor the medical component of the hospice programmiddot Participate in the development coordination and implementation of hospice services and policies middot Participating in the development of staff and volunteer trainingmiddot Conduct home visits in person and virtual as a consulting physician when appropriate middot Act as a hospice liaison and advocate with the national state and local physicians healthcare professionals partners and
