We’re looking for a Hospice LPN to join our team. You’ll report directly to the Administrator or Executive Director and play a key role in providing compassionate, hands-on care to patients and their families. This position supports end-of-life care in the setting patients call home—whether that’s a private residence, assisted living, or facility.
**As a Hospice LPN, You Will:**
+ Provide direct patient care services to hospice patients and supportive care to families with a reasonable caseload
+ Assist the interdisciplinary team in evaluating patient/family needs and contribute to the plan of care
+ Report findings or changes in the patient’s condition to the Registered Nurse
+ Deliver care with empathy and attentiveness in accordance with the patient’s individualized care plan
+ Serve as a valued resource to families, helping them feel supported during difficult moments
**About You**
**Requirements:**
+ Current LPN/LVN license to practice in the state of requested employment
+ At least 1 year of direct patient care experience in hospice, home health, ICU, geriatric, oncology or other high acuity setting
+ Current CPR certification
+ Current driver’s license and automobile insurance
+ Hospice heart: compassionate, kind, and empathetic to patients and families
+ Comfortable providing care in diverse home settings
+ Able to demonstrate flexibility and composure while supporting others during crisis
**Preferred Experience:**
+ Three or more years of clinical nursing experience, especially in hospice or palliative care
+ Background in hospital settings such as Med-Surg, PACU, ER, Float Pool, Critical Care, or Acute Care
+ Familiarity with telemetry, wound care, admissions, cardiac care, or nursing home environments
+ Travel nurse or mobile healthcare experience
