We want your knowledge and expertise! **Job Summary and Qualifications** As a Hospice Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) at HCA Healthcare, you’ll provide compassionate, personalized support to patients receiving end-of-life care – often over the course of several weeks or months. You’ll work with a dedicated multidisciplinary team who cares for patients like family. Together, you’ll bring comfort, dignity, and guidance to patients and their loved ones throughout their hospice journey. **Your responsibilities will include:** + Providing hands-on nursing care in the home or facility by managing symptoms, administering medications, and helping patients stay comfortable + Observing and reporting changes in patient condition to the RN and care team to ensure timely, coordinated care + Documenting patient status, care provided, and important updates clearly and promptly to ensure safety and continuity across the care team + Supporting patients and families with guidance, compassion, and practical education to prepare them for care routines, medications, and end-of-life care + Collaborating with the care team to provide smooth, patient-centered care **What qualifications you will need:** + Basic Cardiac Life Support must be obtained within 30 days of employment start date + Drivers License + (LPN/LVN) Licensed Practical or Vocational Nurse + Vocational School Graduate Occasional/ Intermittent Required 1 years experience Required Years of Experience Hospice & Family Care provides physical, emotional, and spiritual support for patients and families.
