Hospice Clinical Team Manager

What You’ll Do:Oversee clinical operations and lead a team of hospice professionals
Manage patient care coordination, staff assignments, and care planning
Review and approve individualized plans of care and any necessary updates
Partner with physicians and interdisciplinary teams to ensure quality and compliance
Coach and support field clinicians through regular team meetings and one-on-ones
Assist with hiring, onboarding, and performance feedback
Maintain readiness to support in the field as needed
Ensure regulatory compliance, documentation accuracy, and timely care initiation
Monitor performance metrics and report directly to Branch Leadership
What You’ll Bring:RN License (active and unrestricted in the state of practice)Associate degree in Nursing required; Bachelor's degree preferred
2+ years of experience in hospice or home health
1+ year of supervisory or leadership experience
Comfortable navigating systems like Homecare Homebase, Workday, and Microsoft Office
Ability to travel up to 50% to support patient and business needs
Reliable transportation, valid driver’s license, and auto insurance
Physical ability to lift and move up to 50 lbs intermittently
You will report to the Administration or Branch Director.
