What You’ll DoProvide urgent, compassionate nursing care after hours in alignment with Elara Caring’s mission
Assess patient and family needs—physical, emotional, psychosocial, and spiritual
Communicate significant changes or concerns to the Clinical Supervisor, physician, or facility staff
Educate patients, families, and facility staff on hospice philosophy, pain control, and symptom management
Offer support during end-of-life transitions and perform bereavement assessments
Collaborate with the Interdisciplinary Team to ensure the best possible quality of life for each patient
What You BringGraduate of an accredited nursing program (Diploma, Associate, or Bachelor’s)
Current RN license in the state of practice
At least 1 year of clinical nursing experience (hospice or home health preferred)
Willingness to travel up to 50%
Physical ability to lift 50–100 lbs.
Valid driver’s license, insurance, and reliable transportation
