As a Home Health Speech Language Pathologist, you’ll provide patient-centered care that improves communication, swallowing, and overall quality of life.

Evaluate and treat patients using evidence-based speech therapy practicesDevelop and adjust individualized care plans based on patient progressEducate patients and families to support independence and long-term outcomesCollaborate with nurses, therapists, and physicians to coordinate careComplete accurate, timely documentation to support clinical and regulatory requirementsParticipate in care planning, case conferences, and quality improvement initiativesIdentify changes in patient condition and adjust treatment accordinglyWhat We’re Looking ForMaster’s Degree in Speech-Language PathologyActive Illinois SLP license (or eligible)Completion of Clinical Fellowship Year (CFY)Minimum 1 year of experience (or strong clinical internship experience)Strong communication, documentation, and time management skillsAbility to work independently in a home-based settingValid driver’s license and reliable transportationCertificationsBLS Certification (AHA) – required within 6 months of hire (if not already obtained)Work Environment & Physical RequirementsTravel within assigned geographic areaWork in patient homes and assisted living settingsAbility to lift up to 35 lbs (with assistive equipment/team support as needed)Occasional weekends/holidays based on patient needsOur Commitment
