We're hiring a Home Health Physical Therapist – Baylor Shift

What You’ll Do:
Conduct in-home physical therapy evaluations and create individualized treatment plans
Perform OASIS assessments and complete timely documentation
Supervise and collaborate with PTAs and other clinical team members
Deliver evidence-based treatments to help patients regain mobility, strength, and independence
Educate patients, families, and caregivers on treatment plans and safety practices
Participate in case conferences and coordinated care planning
Maintain accurate clinical records and follow Medicare/compliance guidelines
Support transitions of care and refer to other services as needed
Communicate with physicians and internal agency team consistently

Requirements:
Licensed Physical Therapist (Bachelor’s, Master’s, or Doctorate in PT)
Licensed to practice in the state
Minimum of 2 years of clinical experience (home health preferred)
U.S. citizen or Green Card holder
