Schedule: Full-time | Day shift (with weekend/holiday rotation as needed)

This role focuses on delivering individualized therapy interventions, supporting care coordination, and ensuring excellent patient outcomes.
Key ResponsibilitiesPatient-Centered Care
Deliver individualized treatment plans established by the supervising Physical TherapistBuild strong, compassionate relationships with patients and familiesAdapt communication style to meet patient and family needsPromote patient independence with self-care, disease management, and safetyInvolve patients and families in care planning and treatment progressionQuality & Clinical Excellence
Provide evidence-based therapy interventions to achieve patient goalsIdentify changes in patient condition and communicate with supervising therapistMaintain accurate, timely documentation to meet regulatory and reimbursement standardsParticipate in quality improvement initiatives and promote patient safetyCare Coordination
Collaborate with the interdisciplinary team to support coordinated careCommunicate effectively using tools like SBAR to ensure continuityAssist in discharge planning and transition of careParticipate in case conferences and team discussionsProfessional Development & Teamwork

Adhere to organizational policies and productivity expectationsEfficiently manage time, travel, and patient caseloadUtilize resources appropriately to support optimal outcomesEducation & ExperienceGraduate of an accredited Physical Therapist Assistant programMinimum 2 years of clinical experience (or strong clinical affiliation experience)Meets all state continuing education requirementsLicensure & CertificationsLicensed Physical Therapist Assistant in the State of Illinois (or eligible)BLS certification (or obtained within 6 months)Valid driver’s licenseSkills & QualificationsStrong clinical and technical home health therapy skillsExcellent communication and documentation abilitiesStrong time management and organizational skillsAbility to work independently while collaborating with supervising therapistProficiency with home therapy equipment and assistive devicesWork Environment & Physical RequirementsTravel within assigned territory, including varying weather and road conditionsProvide care in patient homes and assisted living settingsAbility to lift up to 35 lbs (with equipment/assistance as needed)Physical activity including bending, climbing stairs, lifting, and patient transfers
