Join our Home Health team as a Licensed Practical Nurse (LPN), delivering compassionate, high-quality care to patients in the comfort of their homes. Working under the supervision of an RN, you’ll play a key role in supporting patient outcomes, coordinating care, and empowering patients and families through education.
Key ResponsibilitiesDeliver skilled nursing care in accordance with individualized care plans under RN supervision
Monitor patient conditions, recognize changes, and communicate updates to the care team
Coordinate care with interdisciplinary team members to ensure continuity and optimal outcomes
Provide patient and family education on disease management, treatments, medications, and safety
Accurately document care to support clinical, regulatory, and reimbursement requirements
Participate in case conferences, team meetings, and ongoing care planning
Promote patient independence and engagement through education and support
Maintain compliance with organizational policies and quality/safety standards
Required QualificationsActive LPN license in the State of Illinois (or eligible)
CPR Certification
Pharmacology certification
Minimum 2 years of Med/Surg clinical experience
Valid driver’s license and reliable transportation
Preferred ExperiencePrior home health experience
Familiarity with Medicare/Medicaid home health guidelines
Skills & StrengthsStrong clinical assessment and critical thinking skills
Excellent communication and documentation abilities
Ability to work independently and manage time effectively
Comfortable using EMR systems and basic Microsoft Office tools
Strong interpersonal skills with a patient-centered approach
Work Environment & RequirementsTravel throughout assigned geographic area to patient homes
Ability to lift up to 35 lbs (with assistance/equipment as needed)
Frequent standing, walking, bending, and patient assistance
Work performed in patient homes and community settings
Compensation💵 $23.60 – $35.30 per hour (based on experience)
