Home Health Coordinator/Scheduler is responsible for ensuring efficient scheduling of patient visits, coordinating staff assignments, and maintaining clear communication between the clinical team. This role supports the overall delivery of high-quality home health services by managing the daily schedule, tracking visit compliance, and ensuring patients receive care in a timely and organized manner. The coordinator acts as the central communication hub for the home health/therapy team, balancing clinical needs, staff availability, and patient preferences.
Pay Range: $17.00 - 20.00 an hour

Develop, maintain, and adjust daily and weekly staff schedules to meet patient care needs.Assign appropriate clinicians to cases based on skills, licensure, and availability.Monitor visit frequency and ensure compliance with physician orders and regulatory requirements.Track cancellations, missed visits, and reschedules, ensuring timely follow-up.Coordinate with intake staff for new admissions and ensure smooth onboarding into schedules.Communication & Support
Serve as the primary point of contact for home health office and therapy staff regarding schedule changes or visit updates.Maintain positive working relationships with home health and therapy team.Operational Support
Provide coverage for incoming referrals, triage scheduling-related inquiries, and escalate clinical concerns as needed.Participate in daily or weekly team meetings to review caseload, staffing, and scheduling priorities.Assist with training new staff on scheduling procedures and systems.Contribute to operational efficiency and patient satisfaction by identifying and implementing scheduling improvements. Qualifications High school diploma or equivalent (associate degree or healthcare-related coursework preferred).
