Responsibilities and Essential Job Functions
Consistently meets work schedule.Demonstrates effective clinical critical thinking skills as demonstrated in application of disease management protocols, pain management interventions and clinical equipment according to department processes and policies.Demonstrates effective management of patient care as evident in patient Plan of Care, medication management, patient teaching and use of visit frequency. Appropriately delegates patient care and performs supervisory visits as indicated.Maintains a safe environment for patients as demonstrated through adhering to fire safety measures and fall risk measures as established by Home Care standards.Accurately documents and maintains all elements of the patient record according to department policies on timeliness and clinical content.Considers patient's age, psychosocial, spiritual and cultural needs to therapeutically meet patient and patient caregiver needs as demonstrated in implementation of patient centered goals and interventions.Participates and seeks out leadership and learning experiences.- Mentors students and new employees.
- Seeks out professional expertise to improve patient care.
- Participates in IDT and department inservices.
- Seeks out resources in efforts to provide solutions in the home setting.Effectively communicates to clinicians, physicians, patients and patient caregivers adhering to departmental process and regulations for all forms communication.Independently performs comprehensive assessment to provide the most effective patient care for a given population in the home environment.Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.These statements are intended to describe the essential functions of the job and are not intended to be an exhaustive list of all responsibilities.
