Facility: BayCare HomeCare Location: Dunedin, FL Status: Per Visit (PRN) Shift Hours: Days Varies, 8:00 a.m. - 5:00 p.m. Weekend Work: Occasional On Call: No Home visits in assigned service area: Dunedin, Clearwater, Palm Harbor, Oldsmar, and surrounding areasThe Home Health Aide -Per Visit provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include:
Working under the guidance and supervision of the staff nurse in providing personal care to the patients
Following the patients' plan of care for completion of tasks assigned to a home health aide by the registered nurse or other appropriate skilled professional
Maintaining an open communication process with the patient, representative (if any), caregivers, and family
Demonstrating competency with assigned tasks; complies with infection prevention and control policies and procedures; reports changes in the patient condition; and honors patient rights
Home Health Aide - Home Care Requirements:
Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
CNA (Certified Nursing Assistant) or HHA certification from accredited 75-hour program
BLS (Basic Life Support)
Preferred: High School Diploma or equivalent
Preferred 1 year Home Care
