Astria Health is seeking a Home Health Aide to join our team!  This is for our Sunnyside location. This is a per diem role. 

 

What you get to do every day!

 

As the Home Health Aide, you will be responsible for the direct personal patient care in a patient’s home according to a patient’s specific plan of care prepared by the RN Case Manager.  You will: 

 

•    Provide personal caregiving services in accordance with an established plan of direct care
•    Provide for the personal needs and comfort of people in their own homes
•    Assist patients with activities of daily living; including bathing, dressing, grooming and toileting
•    Perform general housekeeping activities
•    Provide transportation assistance to and from activities and appointments
•    Offer compassionate companionship to patients
•    Monitor and report change of condition of patient
•    Perform other duties as assigned
•    Collaborate effectively across organizational boundaries.
•    Adhere to the core values and standards of the organization.

High School Diploma Current Washington State Home Care Aid or Certified Nursing Assistant License Basic Life Support (BLS) Oral and written fluency in the English language.
