Assisting clients with daily living activities, including bathing, grooming, dressing and feeding.Assisting patients with mobility and transfers.Completing necessary documentation accurately and promptly.Communicating effectively with clients, families and other healthcare professionals regarding client needs and concerns.Following established care plans and contribute to the development of personalized care strategies.

Job Requirements
High School Diploma or GEDCNA- Completion of 75, 120, or 150 hour Certified Nursing Assistant Pgm/must be in-State C.N.A. Registry.HHA- successful completion of a competency evaluation program.CNA- One year of Certified Nursing Aide-Home Health Hospice.HHA- One year home health aide, patient tech or other relatedExperience and/or passes CHI Health at Home’s competency
