of Duties The provision of intermittent nursing service to patients in their homes under a plan of care established in collaboration with the patientrsquos physician and according to agency policies and proceduresSupervision Received Reports directly to a Clinical ManagerSupervision Exercised Home Care AidesEducation Graduate of an NLN approved school of nursing associate or baccalaureate degree programExperience One year of recent medicalsurgical nursing experience additional experience in community health nursing preferredCertificateLicense Possession of a current license to practice as a Registered Nurse in the State of ConnecticutTypical Physical Demands Requires full range of body motion including handling and lifting patients manual and finger dexterity and eye hand coordination Requires standing walking and the ability to ascend and descend stairs Requires operation of a motor vehicle Occasionally lifts and carries items weighing up to pounds Requires corrected vision and hearing to normal range Requires working under stressful conditions Requires some exposure to communicable diseases andor bodily fluidsTypical Work Conditions Work is performed in patientsrsquo homes communityAbility to maintain quality control standards Ability to react calmly and effectively in emergency situations Ability to interpret adapt and apply guidelines and procedures Ability to communicate clearlyPossess adequate transportation to carry out job requirements Must show proof of satisfactory automobile insurance with State of CT minimum coverage recommended amount of intermittent nursing service to patients and their families in compliance with agency policies and State and Federal Regulations incorporating the followingA complete physical and psychosocial assessmentDevelops and implements an appropriate care plan and therapeutic goals with the patientfamilyEvaluation of patient safety on a hour basisExercises appropriate clinical judgment in coordination of patient care by all services involvedOngoing evaluation of significant changes in the condition of the patient and of progress toward the therapeutic goalsUse of Agency and community consultantsresourcesInstruction of patientfamily in procedures or responsibilities they are assuming and optimum wellness practicesPlanning for the patientrsquos discharge from serviceEstablishes and maintains case mix weight utilization patient satisfaction that aligns with industry standards and agency operational objectives for quality careOASIS documentation correlates with the Plan of CareUses recommended Oasis assessment techniques of the timeOASIS documentation and plan of care correlate to Documents all services provided in the clinical record in accordance with State and Federal Regulations and Agency policyDocumentation including activities and mileage is exported within hours of visit or prior to next scheduled visitObtains physician verbalwritten orders and complies with all reporting requirementsParticipates in coordination of care with physicians and with other disciplines as appropriate Manages work assignment in effective and efficient mannerVisits all new therapeutic clients within hours of referral unless otherwise specifiedby the physicianReports any cases to the Supervisor of Clinical Services where goals can not be metor at risk situations exist and responds appropriatelyRemains flexible in daily planning allowing time for emergencies or unexpectedvisitsMaintains productivity as established by the AgencyUtilizes payer sources correctly reflecting changing levels of careDelegates patient care responsibilities to Home Care Nurse LPN as appropriateDemonstrates an understanding of the principles of case managementDemonstrates ability to manage cost and quality Establishes and evaluates the Plan of Care to be delegated to the Home Health Aide orients and supervises the Home Health Aide in accordance with State and Federal Regulations and Agency policyDevelops written plan of care for the Home Health Aide and updates this plan as patientrsquos condition warrants plan of care must be reviewed every daysObtains input from Home Health Aide regarding patient progressPrepares written performance evaluations of home care aides under hisher supervisionProvides supervision of home care aides at a minimum of every days or as required by payer source Develops and maintains sufficient professional skills and credentials to meet the needs of patient careAttends a minimum of hours of in service per year to include mandated annual in servicesParticipates actively in self appraisal and the formation of a plan of self developmentAdheres to Agency Corporate Compliance Plan and HIPAA Privacy Plan as per written agreementSeeks appropriate clinical input to ensure appropriate patient careProjects professional image through attitude and compliance with dress codeCapable of precepting new nurses and studentsFunctions as a team member with consideration to all staffProvides Agency with current copy of professional licenseMaintains CPR certificationAncillary Duties Performs other duties as necessary to support Agency programs as assigned by the Supervisor of Clinical ServicesAgency committees as requestedFluspecial clinics as neededElder Wellness Clinics as assignedSpecial Agency or community programs during andor after regular working hours Performs other duties as necessary to maintain current Agency personnel policiesGives adequate time and attention to administrative dutiesAbides by Agency weekendholiday rotation
