Trinity Health At Home is seeking a dedicated Home Care/Hospice Coordinator to join our team and help ensure seamless, compassionate care for our clients.
Key Responsibilities
Facilitate referrals from acute care settings to homecare/hospice services.
Educate hospital staff and physicians on available services.
Coordinate care plans and obtain physician orders.
Assess client needs and recommend appropriate services.
Maintain communication with healthcare teams, clients, and families.
Participate in quality improvement and business development initiatives.
Meet sales targets and build referral relationships with providers.
Qualifications
Bachelor’s degree or equivalent experience required.
Clinical education and licensure preferred.
Minimum 2 years’ experience in acute care, customer service, or sales/marketing.
Valid driver’s license and reliable transportation.
Strong communication and organizational skills.
Commitment to ethical standards and continuous improvement.
