Responsibilities include coordinating patient care through all phases of transplantation which include pre-operative, peri-operative, and postoperative care and follow up. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. **The value you'll bring to the team:** + Participates in 24-hr on-call schedule + Facilitates/coordinates individualized evaluations for potential organ recipients or living donors which includes: collection of patient medical data, patient/family education, identification of co-morbidities/contraindications to transplant (or living donation), collaboration with transplant team members. + Coordinates patient care in outpatient setting to achieve optimal healthcare recovery. * Reinforces patient education * Monitors/assesses patient for potential post-transplant complications. + Coordinates the entry and the integration of patients into the program by facilitating appointments for tests, procedures, and required family services. Coordinates and expedites all BMT protocols and informed consents. Communicates and collaborates with multi-disciplinary team and other healthcare personnel relevant to the program and the status of upcoming patients.
