What You'll Do + Collaborate with the Heart Failure team in all aspects of patient care, coordinating care needs of the heart failure population from the time of referral through end-of-life. + Provide disease self-management education and serve as a resource for heart failure patients. + Actively participate in performance improvement initiatives to enhance care, processes, and outcomes. + Triage patient phone calls and collaborate with medical providers to titrate medications within scope of practice. What We're Looking For + Education: 4 Year / Bachelor's Degree, prefered. BSN, preferred. + Experience: 3 years of experience in Telemetry, CCU, Cardiac Stepdown; Minimum of 3 years of Clinical RN experience, required. + Licensure/Certification: Registered as RN in NC or compact state, required. Certification as a CHFN, required. + Additional Skills Required: + Additional Skills/Requirements (required) + Demonstrates interdisciplinary leadership skills, superior interpersonal communication skills, and able to communicate patient’s needs to providers. + Organization skills, critical thinking skills, and clinical experience/expertise in HF management + Ability to work independently on their own as well as collaborative with the interdisciplinary team.
