Duties Description With an eye toward continuous quality improvement, the incumbent will serve the Metropolitan Regional Office on our Central Islip team as the premier social work services expert and in this capacity, will conduct routine surveys in Adult Care & Assisted Living facilities throughout the region, which includes reviewing resident records, staff and resident interviews, reviewing facility-developed materials for appropriateness, and collecting evidence to support findings. There is an occasional need for an off-hour on-site presence and the incumbent may be required to go on-site during these times to ensure resident safety. In addition, the incumbent will investigate complaint allegations & facility submitted incident reports regarding non-compliance with programmatic and resident rights related regulations. Complaint investigations are done either independently or as part of a team, depending on the nature & scope of the allegations. As needed to support licensure of new providers and/or to support regulatory compliance, the incumbent may be assigned review of provider-developed material including, but not limited to, regulatory waivers, policies, and procedures, and plans of correction, for regulatory accuracy and to support the provider’s compliance with environmental requirements. The incumbent will be responsible for investigating incidents and complaints as well as writing and reviewing reports issued to the Justice Center for the Protection of People with Special Needs. They will be required to be familiar with the Vulnerable Persons Central Register and regulations associated with facilities under the joint jurisdiction of the Department and Justice Center. In this team oriented-environment, the incumbent will support emergency preparedness and response activities, as well as initiatives underway in other long-term care programs to support the overall mission of the Department and/or the Office of Aging and Long-Term Care as needed.
Minimum Qualifications Permanent Transfer Candidates: Current NYS Department of Health employee with one year of permanent or contingent-permanent, competitive, status as a Health Systems Specialist 2 (Social Work), G20 OR current NYS employee with one year of permanent or contingent-permanent, competitive, status in a title G18 or higher and eligible to transfer under Section 70.1 of the NYS Civil Service Law.This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS).For the duration of the NY HELPS Program, this title maybe filled via a non-competitive appointment, which means no examination is required, but all candidates must meet the minimum qualifications of the title for which they apply. At this time, agencies may recruit and hire employees by making temporary appointments. In June 2024, if a temporary NY HELPS employee is satisfactorily performing in the position, the appointment will be changed from temporary pending Civil Service Commission Action to permanent non-competitive and the official probationary period will begin.At a future date (within one year of permanent appointment), it is expected employees hired under NY HELPS will have their non-competitive employment status converted to competitive status, without having to compete in an examination. Employees will then be afforded with all of the same rights and privileges of competitive class employees of New York State. While serving permanently in a NY HELPS title, employees may take part in any promotion examination for which they are qualified.Non-Competitive Candidates Via NY HELPS: Possession of New York State license and current registration as a Licensed Master Social Worker AND one year of post-licensure experience providing clinical services in a licensed healthcare facility or program or utilizing clinical knowledge in the review and oversight of clinical services for quality assurance or reimbursement purposes; or registration as a Licensed Clinical Social Worker.Preferred Qualifications: Experience with providing case management services in an Adult Care Facility or care management along the long-term care continuum. Ability to evaluate records against applicable laws and regulations. Ability to draft and/or review Statements or Notices of Deficiencies, Plans of Correction, Investigative reports and survey information against statewide standards for consistency and quality assurance. Strong organizational skills and proficiency with the Microsoft Office Product Suite. Ability to gather data as needed for reports, recommend enforcements or sanctions. Excellent communication skills, both oral and written. Federal Surveyor Minimum Qualifications Test certification.
Additional Comments This position is eligible for a downstate adjustment of $4,000. This position may require travel throughout New York State, including overnights to areas not served by public transportation. The incumbent must pass the Surveyor Minimum Qualifications Certification Test (SMQT) within 1 year of appointment. The successful incumbent must be flexible and available to work varied hours, which may include weekends and holidays, to respond to emergencies and/or provide guidance relative to off-hours investigations, as needed.
