Community Health Navigator is responsible for providing outreach education health assessments and community navigation services to identify recruit enroll and screen qualified program participants The position is grant funded through the Department of Public Healthrsquos CT Early Detection Prevention Program CEDPP The position is contingent upon continued funding Responsibilities include but are not limited tomiddot Provide health education outreach activities to individuals in eligibility priority populationsmiddot Collaborate with local community agencies to identify potential underserved participantsmiddot Enroll eligible participants into the CT Early Detection Prevention Program CEDPPmiddot Assess eligible participants for barriers preventing participation in early detection screening and prevention servicesmiddot Complete all required assessments screenings and documentationmiddot Work closely with medical providers and clinical personnel to ensure that clients have integrated comprehensive coordinated healthcaremiddot Attend all required program meetings DPH provider meetings and training sessionsmiddot Display and uphold HHC core values of dignity compassion service excellence community and integrity Consistently demonstrates caring for patients for one another and for the organization they are part of and contributes to building trust pride and license and reliable transportation requiredmiddot Basic clerical and personal computer skills including Microsoft Office suite are requiredmiddot Flexibility in work hours including some availability evenings and weekends when neededmiddot Ability to work independently and communicate effectively via phone video conferencing and in person a mustmiddot Ability to manage multiple priorities and
