Health Coordinator

The Health Coordinator ensures the delivery of comprehensive health services for enrolled children ages birth to five and pregnant mothers. Collaborates closely with staff, families, community partners, and healthcare providers to ensure timely screenings, referrals, and follow-up care for enrolled children. Oversees health tracking systems, guides staff and families on best practices for child wellness and supports compliance with required documentation and reporting. Provides health education and training to staff and parents and fosters a culture of health, safety, and wellness throughout program.

Candidate qualifications:

Bachelor’s degree in public health administration, Social Service or a related field required. A minimum of one year of relevant work experience in early childhood or health services. Experience working with low socio-economic and culturally diverse families required. Ability to communicate in both English and Spanish preferred. 

Job Responsibilities: 

Works in partnership with community partners and various internal and external stakeholders to bring resources and support to programs and identify opportunities for new community partnership agreements. Oversees and monitors well-child exams, preventative oral health, dental treatment, hearing and vision screenings and follow-up, immunizations, management of chronic conditions, treatment or follow-up testing, health insurance and medical/dental Home. Oversees data efficacy for health data entry in the ChildPlus data management system for clients served including well-child exams, preventative oral health (dental exams and cleaning w/ fluoride), dental treatment, hearing and vision screenings and follow-up, immunizations, management of chronic conditions, treatment or follow-up testing, health insurance and medical/dental Home. Coordinates and documents internal staffing for children with health or medication needs and concerns. Follows and adheres to milestones and internal monthly monitoring and reporting. Ensures family support specialists, home visitors and nurse consultant understand and implement their role in the health services system which includes health case management. Ensures accurate lists of under-immunized children, allergies, and other chronic health conditions.

Compensation: Hourly; Range: $24.00-$26.00 per hour offers are commensurate with experience and bilingual candidates may receive additional compensation. The salary range provided represents our current estimate for this role at the time of posting and is subject to change. The final salary will be determined based on a range of factors, including but not limited to, a candidate's relevant experience, education, qualifications, skills, certifications, and alignment with organizational needs.
