Health Advocates play an essential role on the interdisciplinary care team and are responsible for telephonic outreach to members to inquire about current state health condition.  Health Advocates will review available data and coordinate care with the clinical team as members’ condition change.  Health Advocates assist members with health care appointment scheduling and locating local resources to assist with member non-clinical needs.   Additional responsibilities include outreach to members to inform them of the value participating in the Kidney Services program will provide them and scheduling outreach appointments with the clinical team using a scheduling tool. 

 

This role is a fully remote position.

Accurately manages multiple technology systems simultaneouslyAccurately documents interaction and appropriately follow all processes and procedures related to engagementBuilds rapport and establishes trusted relationships with members via daily outbound and inbound callsProvides excellent customer serviceMedical appointment scheduling assistance
Qualifications

Required Qualifications:

High School Diploma / GED OR equivalent work experienceMust be 18 years of age OR older2+ years of prior medical / healthcare experience from one or more of the following: nursing, hospital, clinic, healthcare insurance, ER, EMT, long - term care or telephonic healthcare consultingAbility to create, edit, copy, save, and send documents, spreadsheets, and correspondence utilizing Microsoft Word, Microsoft Excel, and Microsoft OutlookAbility to navigate a PC to open applications, send emails, and conduct data entryAbility to work 40 hours per week Monday - Friday 8:00am - 5:00pm with 2-3 days weekly covering the later hours of 5:00pm - 8:00pm local time.  Flex scheduling available when working later hours.

Preferred Qualifications:

Bilingual fluency in English and Spanish1+ years of telephonic customer service experiencePrevious CKD (Chronic Kidney Disease), cardiac / diabetic knowledge or experienceHealthcare or insurance benefits knowledge and/or experienceExperience with EMR systems
