**Job Description**
To provide clinical counseling services to patients and families of the Addiction Institute Halfway House. Is cognizant of the philosophy, standards, objectives and policies of the Department and the Hospital Center.
**Qualifications**
+ Bachelor’s degree required. Master’s degree preferred
+ A minimum of two years’ experience as a supervisor.
+ Competencies of the position, counselor/supervisor.
+ Active New York State Licensure in Alcoholism and Substance Abuse Counselling (CASAC),
+ Social Worker (LCSW OR LMSW) Mental Health Counselling (LMHC) preferred.
**Responsibilities**
A. CLINICAL/ TECHNICAL/ SERVICE
• Demonstrates the ability to perform clinical/ technical/ service/ administrative tasks
• Attends and participates in team meetings
• Prepares intake psychosocial, initial treatment plan, recovery and discharge plans for patients on caseload
• Does individual and group counseling
• Documents treatment in medical record via EMR systems
• Presents at case conferences
• Manages the care assigned to patients on caseload
• Participates in clinical supervision and utilizes direction
• Provides reports of clinical services to supervisor when requested
• Make referrals to appropriate resources for the ongoing care of patients
• Provides support counselling, including at least bi-weekly appointment with each patient on caseload
• Interfaces with managed care companies and referral sources to implement Treatment Plans
• Provides consultation to patients throughout Mount Sinai Hospital when requested
• Supervises and collects urine specimen for toxicology
• Assess the need for psychiatric services and make appropriate referrals
• Provides intensive case management for patients with multiple problems when necessary
• Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria appropriate to the age of the patients served in assigned area
• In the absence of Halfway House Manager: maintains contact with OPD, exercise control of petty cash, patient allowances for distribution, arrange discharges referrals, and conducts exit interviews.

• Identifies learning strengths and needs
• Utilizes learning resources
• Presents at in-service training
• Delivers lectures at workshops sponsored by Addiction Institute
D. COMMUNICATION/ RELATIONSHIPS
• Demonstrates a professional, courteous, and respectful attitude in dealing with patients
• Displays courtesy, tact and patience during interactions with all members of the hospital staff and extended community
• Communicates effectively with managed care, insurance companies, EAP’s, and other current or potential referral sources

**Compensation Statement**
Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $59267 - $88900 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
