Habilitation Coordinator I

The Department of Developmental Services (DDS) seeks a caring and compassionate applicant for the role ofHabilitation Coordinator I. The Habilitation Coordinator I will coordinate with the Residential homes in CCS (Commonwealth Community Services) Programs. This candidate will need to have an understanding of community integration. This position would look to increase individual and staff knowledge of opportunities within the community. Have the ability to assess the needs of the individual by building trust and providing non-judgmental support to link them to their preferred activities to facilitate creating meaningful relationships. Provide continued guidance and oversight throughout the process and fade involvement once staff and individuals are confident but remain as a resource. Support staff in developing within the home meaningful activities for those who are retired or lack a structured work/day program.

Duties and Responsibilities (these duties are a general summary and not all inclusive):

Assists in assessing individuals’ needs in conjunction with available resources and assists with the development of goals and plans with an emphasis on community membership, fostering relationships, and promoting individual choice. Acts as a liaison between community residential services and community organizations, social services agencies, groups and individuals to educate staff and the community on the philosophy of the program and ensure positive membershipParticipates in the evaluation, implementation, and monitoring of integration plans, as requested/assigned, to promote consistency and to model implementation for other staff. Participates as an ISP team member, completes necessary documentation, and attends team meetings as appropriate/assigned. Participates in residential management process as a clinical resource; assists in developing strategies to facilitate individuals’ community membership and integration as defined by current quality assurance processes. Participates in projects as assigned; provides accurate documents to Program Directors regarding services provided; provides input to management staff as requested. As assigned, provides values based training to residential and clinical staff on a regular basis. Attends related workshops and conferences to maintain and increase professional development. Provides direct services to individuals within CW and develops integration plans prior to fading to residential staff based on residential referral. Facilitates in-home training sessions regarding person-centered work, including community integration. Provides continued guidance and oversight throughout the process and fades involvement once staff and individuals are confident. Remains as a resource for staff. Facilitates person-centered planned sessions for identified individuals.

Preferred Qualifications:

Medication Administration Program (MAP) certification preferred but not required.Teaching certification and Certified Therapeutic Recreational Therapist (CTRS) preferred, but not required.

Please Note:

This position is located in Northampton, MA and provides coverage throughout the Commonwealth Community Services Program.

Schedule: 9:00am - 5:00pm

Days Off: Sunday/Saturday

As part of daily assignments, the incumbent will be required to drive between work sites and to perform other related duties as required.This may include participation in professional meetings, workshops, or seminars to keep abreast of changes in professional practice.
