Grief Group Facilitator

**Job Description:**
Responsible to provide a program of supportive services to help meet patient/family emotional needs regarding issues of loss and grief. Works collaboratively with interdisciplinary team members to coordinate an anticipatory grief and bereavement follow-up plan of care.Responsible to facilitate grief support groups, providing psychosocial support and resources to those who are impacted by life-threatening illness or who are bereaved. •Cofacilitate community support groups using evidence based models of grief •Refer support group members to a higher level of care as appropriate
**Education Qualifications:**
Bachelors degree required with course work or experience in loss and grief.
Qualified applicants must complete a 2 day Bereavement Group Facilitator Training provided by Essentia Health Grief Support Services.
