Innovation.** **Job purpose** The Field Nurse Case Manager (FNCM) will play an essential role in supporting patients living with a dermatological disease to improve both their health outcomes and quality of life. They will report to the Senior Manager of Patient Support Programs within the Medical Affairs department. FNCM will be accountable for day-to-day nursing and administrative operations of the patient support program (PSP), general oversight within their assigned territory. The successful candidate will represent Sun Pharma and work closely with the designated PSP. **Duties and responsibilities** FNCM offers enrolment and reimbursement support to physicians and medical case management services to patients. Assist the physician with the completion of program-specific paperwork and follow-up on submissions pertaining to obtaining drug reimbursement/coverage through private and provincial drug formularies to ensure patients have access to their prescribed treatments in a timely manner. Act as a key resource to physicians who are prescribing this medication and continuous support for renewals, and coverage changes. + Educate physicians and/or their staff on various PSP services + Liaise with physicians offices, pharmacies, service providers and other parties to provide comprehensive and coordinated service and resolve issues as they arise. + Provide therapy guidelines to manage physician expectations with respect to reimbursement + Assist MDs in completing Special Authorization (SA) forms, enrolment forms, prescription renewals, and other documentation + Provide ongoing support and updates to physicians on their patients + Customize current program elements to meet the needs of assigned physicians: adjust based on in-clinic and remote MD needs. + Provide product support + Participate in scheduled physician meetings with Sun Pharma or PSP personnel + Regular travel to various physician offices + Responsible for defined key performance indicators with in the given territory + Acts as a liaison between PSP and external clinical staff + Participate in quality assurance activities and continuous improvement initiatives + Provide in-person and virtual support to internal and external stakeholders. + Work directly with PSP and clinic to secure funding for patients therapies + Adhere to Performance Standards and Procedures required by SUN and the PSP **Additional responsibility (Only applicable to customer facing roles)** Report any adverse event received from customers for company products to the pharmacovigilance department / Drug Safety on the same day or within 1 business day **Qualifications** + Graduate of Registered Nursing Program or Registered/Licensed Practical Nursing program holding current registration for Quebec province. + Minimum 2 years experience in Patient Support Programs or equivalent + Ability to engage with a physician and their staff as a trusted and well-informed medical counterpart + Ability to resolve conflict in a professional manner + Strong sense of organization and attention to detail, accurate data collection + Self-regulation of time management and the ability to multi-task and adhere to deadlines + Familiarity with PIPEDA and how it applies in a confidential patient environment + Ability to work in a quiet working environment with proven ability to work autonomously within a remote/virtual team environment + Adhere to Performance Standards and Procedures required by the PSP (Patient Support Program) + Previous Case Manager experience an asset + Understanding of provincial and private coverage criteria, Special Authorization processes and how to navigate the Canadian drug coverage landscape is an asset + Strong critical thinking and problem-solving skills + Strong computer and technical skills: Excel, Word, Outlook, Tablets + Must be fluent in verbal and written communication in French and English **Working conditions** + Field based out of Montreal + Must have a private, dedicated home office space with high speed internet access that accommodates equipment supplied by Sun Pharma Canada. + Must have a valid drivers license, a personal car with the capability and willingness to travel extensively across assigned territories, utilizing reliable personal transportation **Physical requirements** + Ability to travel, with overnight stays as needed **Direct reports** NA **Objectif du poste (Job Purpose)** L'infirmier (ire) de terrain charg(e) de la gestion des cas aura un rle essentiel dans le soutien apport aux patients atteints de maladies en dermatologie afin d'amliorer leur tat de sant et leur qualit de vie. Il/elle se rapportera au Gestionnaire en chef des programmes de soutien aux patients au sein du dpartement des affaires mdicales. **Tches et responsabilits (Duties and responsibilities)** L'infirmier(ire) de terrain supporte les mdecins avec l'inscription au Programme de support aux patients (PSP), le remboursement des mdicaments et les services de gestion des dossiers mdicaux aux patients. Aider le mdecin remplir les documents propres au programme et assurer le suivi des demandes relatives l'obtention du remboursement ou de la couverture des mdicaments par le biais des formulaires privs et provinciaux, afin que les patients aient accs aux traitements qui leur sont prescrits en temps opportun. Agir en tant que ressource cl pour les mdecins qui prescrivent ce mdicament et apporter un soutien continu pour les renouvellements et les changements de couverture. + duquer les mdecins et/ou leur personnel sur les diffrents services du PSP + Assurer la liaison avec les cabinets mdicaux, les pharmacies, les prestataires de services et d'autres parties afin de fournir un service complet et coordonn et de rsoudre les problmes au fur et mesure qu'ils se prsentent. + Fournir des directives thrapeutiques pour grer les attentes des mdecins en matire de remboursement. + Aider les mdecins remplir les formulaires d'autorisation spciale, les formulaires d'inscription, les renouvellements d'ordonnance et d'autres documents. + Fournir aux mdecins un soutien continu et des mises jour sur leurs patients. + Personnaliser les lments du programme actuel pour rpondre aux besoins des mdecins assigns : ajuster en fonction des besoins des mdecins en clinique et/ou en rgions. + Apporter un soutien sur les produits + Participer aux runions planifies avec les mdecins et les reprsentants des ventes Fournir aux mdecins des mises jour rgulires sur leurs patients + Visites frquentes de divers cabinets mdicaux. + Responsable des indicateurs de performance cls dfinis dans le territoire donn + Assurer la liaison entre les PSP et le personnel clinique externe. + Participer aux activits d'assurance qualit et aux initiatives d'amlioration continue. + Fournir un soutien en personne et virtuel aux parties prenantes internes et externes. + Travailler directement avec le PSP et les cliniques pour obtenir le financement des thrapies des patients. + Respecter les normes et procdures de performance exiges par SUN et le PSP. **Responsabilit supplmentaire (uniquement applicable aux rles en contact avec les clients) (Additional responsibility (Only applicable to customer facing roles))** + Signaler tout vnement indsirable reu des clients concernant les produits de lentreprise au dpartement de pharmacovigilance / Scurit des mdicaments le jour mme ou dans un dlai dun jour ouvrable. **Qualifications** + Un diplme en soins infirmier provenant dune cole reconnue (Infirmier (ire)/Inf. auxiliaire). + tre inscrit (e) et en rgle auprs de lorganisme de rglementation de sa province. + Au moins deux ans dexprience dans les programmes de soutien aux patients ou lquivalent. + Capacit s'engager auprs d'un mdecin et de son personnel en tant qu'homologue mdical de confiance et bien inform. + Sens aigu de l'organisation et souci du dtail, collecte de donnes prcises + Auto-rgulation de la gestion du temps et capacit effectuer plusieurs tches la fois et respecter les dlais. + Connaissance de PIPEDA/ LPRPDE et de la manire dont les rgles sappliquent dans un environnement confidentiel pour les patients. + Aptitude travailler dans un environnement calme et capacit avre travailler de manire autonome au sein d'une quipe distante/virtuelle. + Respecter les normes de performance et les procdures requises par le PSP. + Une exprience antrieure en tant que gestionnaire de cas est un atout. + Comprhension des critres de couverture provinciaux et privs, des processus d'autorisation spciale et de la faon de naviguer dans le paysage canadien de la couverture des mdicaments est un atout. + Solides comptences en matire de pense critique et de rsolution de problmes + Solides comptences informatiques et techniques : Excel, Word, Outlook, tablettes + Capacit rsoudre les conflits de manire professionnelle + Matrise du franais et de l'anglais, l'oral comme l'crit. **Conditions de travail (Working conditions)** + Bas sur le terrain Montral + Doit disposer d'un bureau priv domicile avec accs l'Internet haute vitesse qui convient l'quipement fourni par Sun Pharma Canada. + Doit avoir un permis de conduire valide, une voiture personnelle, la capacit et la volont de voyager dans les territoires assigns, en utilisant un moyen de transport personnel fiable. **Exigences physiques (Physical requirements)** + Capacit voyager, avec des sjours de nuit si ncessaire **Subordonns directs (Direct Reports)** N/A We provide equal employment opportunities for all current employees and applicants for employment.
