Clinic in Stratford CT is connected to our Hartford Healthcare Urgent Care center It is part of a multidisciplinary team of medical rehabilitation and wellness professionals dedicated to improving the health of all patients This therapist will collaborate with professionals throughout the facility to support our coordinated care model All of our clinics have ample private treatment spaces and state of the art gyms to support our patientrsquos recovery Specialty Programs in this multi disciplinary clinic include Balance Falls Safe Steps Cancer Rehabilitation Complex Spine Concussion Dry Needling Gender Affirming Rehab Hand Therapy Lymphedema StrokeBrain Injury and Spinal Cord Injury Parkinsonrsquos and Movement Disorders Chronic Pain Management Pelvic Health Speech Therapy Sports Medicine Thoracic Outlet and Vestibular TherapyResponsibilitiesThe Exercise Physiologist implements and supervises physical exercise programs for patientsmiddot Provides patient testing and assessment before and after rehabilitationmiddot Plans and teaches exercise therapy for patients utilizing test data patient status and currentmedication regimen Monitors patients response and revises plan accordinglymiddot Recognizes contraindications and abnormal patient responses to exercise Responds appropriately to allsituations including exercise cessation and referral to medical professionalsmiddot Provides exercise information for patients peers and the community which may include public speakingand education classesmiddot Collaborates with physician regarding the exercise prescription for each patient Maintains in depthknowledge of monitoring and arrhythmia identificationmiddot Preparation of report on individual patient to appropriate physician Appropriately charts
