Description A TEKsystems client has an immediate opening for an experienced Epic Certified Healthy Planet Analyst with a new Healthy Planet implementation. The Epic Healthy Planet Analyst is responsible for the design, build, testing, implementation, and ongoing support of Epic’s Healthy Planet module within the organization’s EHR system. This role focuses on population health management, care coordination, and analytics to improve patient outcomes and meet regulatory requirements. Key Responsibilities: + System Configuration & Build: + Configure and maintain Epic Healthy Planet workflows, registries, and reporting tools. + Develop and optimize dashboards, care gap alerts, and population health metrics. + Integration & Data Management: + Collaborate with clinical and operational teams to ensure accurate data integration from multiple sources. + Validate data integrity and troubleshoot issues related to registries and reporting. + Testing & Implementation: + Conduct unit and integrated testing for new features and upgrades. + Support go-live activities and provide post-implementation support. + Stakeholder Collaboration: + Work closely with clinicians, care managers, and IT teams to align system functionality with organizational goals. + Provide training and documentation for end-users.
