Job Title: Enrollment Specialist
Ministry: Trinity Health PACE
Location: St Francis
Position Purpose:
As an Enrollment Specialist, you will serve as the first point of contact for newly qualified referrals, introducing the PACE model of care and evaluating initial eligibility. You’ll play a critical role in managing the full enrollment process while building strong relationships with prospective participants, families, and community partners.
What You Will Do:
Promote the PACE program to prospective enrollees and community agencies.
Collect and document demographic, financial, and clinical eligibility information.
Coordinate all phases of the enrollment process using CRM and EHR systems.
Assist with Medicaid applications and communicate with external agencies.
Support sales initiatives through community events and outreach.
Foster collaborative relationships across interdisciplinary teams.
Minimum Qualifications:
Associate degree and at least two years of sales or marketing experience preferred (or equivalent combination).
Valid driver's license and reliable transportation.
Strong communication and organizational skills.
Proficiency in Microsoft Office; CRM/EHR experience preferred.
Must meet applicable state health requirements prior to employment.
