Educational Coordinator - Critical Time Transition Program

Job Summary
The Educational Coordinator is responsible for ensuring that youth served by the Critical Time Transition Program (CTTP) receive individualized, appropriate, and legally compliant educational services. Most youth referred to CTTP have existing Individualized Education Programs (IEPs) that require refinement or revision to meet their current developmental, clinical, and academic needs. This role includes close collaboration with local school districts and Committees on Special Education, and strong advocacy for services such as tutoring, virtual learning, and transportation. Many of these services are high-cost or logistically complex, requiring skilled coordination and persistence. Some youth may be involved with Child Protective Services or placed in the hospital without an identified caregiver at the time of referral, further increasing the need for assertive educational advocacy.
Essential Job functions
+ Serve as the primary liaison with school districts and Committees on Special Education for all educational planning and placement decisions.
+ Review and support timely updates to Individualized Education Programs based on clinical assessments and youth progress.
+ Advocate for and help arrange educational services including home tutoring, remote instruction, and in-person learning with transportation when clinically appropriate.
+ Attend and contribute to Committee on Special Education meetings, supporting eligibility determinations, placement decisions, and service implementation.
+ Coordinate with internal clinical and care coordination teams to ensure educational services are aligned with treatment and discharge planning.
+ Monitor delivery of educational services, track progress, and intervene when gaps or delays are identified.
+ Engage with caregivers, caseworkers, and systems partners, including Child Protective Services and local Departments of Social Services, to support educational stability and access.
+ Maintain thorough and timely documentation of all educational communications, plans, and advocacy efforts.

Education & Experience
+ Bachelor’s degree in Special Education, Social Work, School Psychology, or a related field required. Master’s degree preferred.
+ Minimum 3 years of experience working with students who have Individualized Education Programs, preferably in coordination, case management, or advocacy roles required.
Special Requirements
+ Unrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.
+ Must be able to work 12-month schedule

+ Strong working knowledge of the Individuals with Disabilities Education Act and New York State special education regulations and procedures.
+ Demonstrated experience advocating for high-cost or intensive educational supports.
+ Excellent communication, organization, and documentation skills.
+ Demonstrated experience working with youth with complex behavioral health and developmental needs.
+ Familiarity with child welfare systems and processes.

+ Must be able to work a variable scheduling including evenings and weekends
+ The following physical demands are required:
+ Occasional sitting (up to 4 hours)
+ Occasional standing (up to 1 hour)
+ Occasional walking (up to 2.5 hours)
+ Occasional driving (up to 8 hours)
+ Continuous balancing (up to 8 hours)
+ Occasional balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching forward or down, reaching overhead, running, and twisting (up to 2.5 hours)
+ Weekly lifting up to 50 lbs. (community-based staff may be required to lift an empty wheelchair weighing up to 50 lbs.), 10-15 lbs. on a daily basis
+ Weekly carrying up to 55 lbs. approximately 10-100 ft., 10-15 lbs. on a daily basis
+ Manual dexterity is required, including the frequent ability to grasp in both hands and continuous use of fine manipulation skills in both hands (approximately 1-2.5 hrs.)
+ Occasional exposure to dust, fumes, gases or chemicals is apparent.
+ Protective clothing or equipment required: Close toed shoes; Protective Shields, Universal Precaution PPE, Gloves for toileting and food preparation
+ Ability to change positions as needed
$60,500 Minimum pay rate, $75,000 Maximum pay rate, based on experience.
