Job Summary
Schedule: PRN; Hours available any day of the week between 8:00 AM - 6:00 PM; must be available to train during the week for several weeks
Location: Charlotte Orthopedic Hospital - Charlotte, NC; after 6 months will move to a hybrid schedule

Responsibilities
+ Serve as facilitator of the patient throughput/discharge planning process
+ Collaborate with case managers and other members of the healthcare team to implement the discharge plan agreed upon by the RN, case manager, physician, patient/chosen support system, and other involved members of the interdisciplinary team
+ Support a wide variety of operational support functions that support discharge planning activities
Qualifications
+ Education: High School Diploma or GED, required.
+ Experience: Health Care experience, Case Management or related experience, knowledge of Microsoft office products/keyboarding, required.
+ Additional Skills (required): Weekend and holiday coverage. Ability to travel between facilities. Excellent interpersonal and customer service skills. Working knowledge of personal computers and ability to learn a variety of software programs. Proficient spelling and grammatical skills. Attention to detail with excellent planning and organizational skills. Communicate effectively in both oral and written forms. Ability to operate standard office equipment including, but not limited to, a calculator, fax machine, copier, printer and scanner. Ability to successfully maintain effective work relationships. Ability to maintain organization and multi-task.
