On-call from 12:30am - 8am

*This position will have all triage and afterhours staff reporting to them*

Position Summary

Provides oversight of all patient care services and personnel. Oversight must include the following: Making patient and personnel assignments, coordinating patient care, coordinating referrals, ensuring patient needs are continually assessed, ensuring development, implementation, and updates of the individualized plan of care.

Coordination of appropriate patient assignments. Coordination of lab results to all physicians. Review of required documentation, facilitates interaction between field staff, office staff, the home care patients, and their providers.


Position Specific Responsibilities

• Provides expertise and support to Clinical Director in developing, evaluating, and revising home care policies, procedures, standards, competencies, quality and risk management, and objectives to ensure clinical operations and processes meet industry standards, regulatory requirements, and the objectives of the agency.

• Manages clinical staff to ensure efficiency, accuracy and quality and makes provisions for their development including in home supervisory visits as needed.

• Works with Education team to provide oversight of orientation of new employees.

• Develops and monitors improvement plans and delivers coaching and/or corrective actions.

• Works with HR to investigate performance issues and respond appropriately.

• Ensures high quality care (HRO) is consistently delivered via multidisciplinary patient care while meeting state and federal regulations by identifying needs, implementing plans, monitoring results and utilizing managerial and interpersonal skills.

• Coordinates with other agency programs to facilitate continuity of care.

• Responsible for oversight of referral management to optimize safety and quality of care, to include availability, visit schedules, staffing levels and geographic areas to maximize efficiency and continuity of services.

• Develops and maintains clinical staffing plan.

• Reviews recommended new positions to respond to increased patient care visits and processes posting upon approval.

• Manages submittals and requisitions for staff vacancies, interviews and hires clinical and support staff.

• Coordination in the maintenance and updating of competency records, credentials and annual monitoring of clinician personnel file requirements.

• Manages clinical staff for compliance with required items such as HealthStream assignments, CPR, auto insurance, driver's license, licensure, TB and flu shot acceptance/denial, etc.

• May delegate this responsibility as appropriate.

• Responsible for administration of client care services.

• Responds to questions, concerns or complaints from clients and other customers related to agency services and documents such interactions as per agency policy/procedure.

• Provides a positive and supportive environment for customers.

• Demonstrates commitment to continuous Quality improvement and collaborates with staff in the activities related to that program.

• Regularly seeks best practices and utilizes process improvement principles to ensure processes and procedures are efficient, cost-effective, and deliver high quality results.

• Participates on the Performance Improvement Committee (QAPI), review data, trends information and recommendations for improvement.

• Functions as a patient advocate: Exercises cultural sensitivity when dealing with a diverse patient, family and guest population in regards to sex, race, religion, age, sexual orientation, marital status, culture, etc.

• Provides and contributes to a positive working environment to implement and reflect the mission statement of the organization through the Core Values.

• Maintains confidentiality of all patient demographics, including medical and financial information at all times.

• Maintains confidentiality of all company information.

• Ensures proper handling and disposal of confidential documents and adheres to all HIPAA rules and regulations.

• Implements disaster/triage plan per protocol after hours, weekends and holidays as directed by management.


Education and/or Experience

CA:
Preferred - Bachelor's Degree Clinical SpecialtyCA:Preferred - 3 years Supervisory/management experience.Required - 3 years Home Care experience.


Skills

Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.Language Skills: Strong verbal and written communication skillsComputer Skills: Ability to use office equipment including computers, telephones, copy and fax machinesOther Skills and Abilities: Ability to understand, read, write, and speak English. Certifications, Licenses, and Registrations

CA:Preferred - 3 years Supervisory/management experience.Required - 3 years Home Care experience.


Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential. Compensation range: $63.52 - $98.60 

  

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities
