Crouse’s Care Coordination team is hiring a Director of Care Coordination.
Pay Range: $150,000.00 - $180,000.00 / annual
Schedule: Full Time, Monday-Friday, 8:00am-4:30pm.
Director of Care Coordination Responsibilities:
Develops internal systems utilizing appropriate resources to support the operating needs of the assigned department(s) of responsibility including, but limited to: finance, planning, marketing, advertising, information systems, human resources and other support systems. Coordinates financial and budgetary acquisitions, allocations and utilization of resources needed for quality, cost-effective, program(s)/service(s). Provides strategic guidance to leadership staff within all areas of responsibility. Develops, plans, and evaluates short, mid and long-term goal setting and goal achievement through facilitation and guidance consistent with the strategic plan. Maintains current industry knowledge serving as a subject matter expert; ensures compliance with all federal/state/local laws, regulatory/accrediting guidelines and professional standards within all areas of responsibility. Attends and actively participates in hospital and performance improvement councils and committees as requested. Chairs the Continuum of Care Performance Improvement Council. Proactively communicates with staff and keeps them informed on a regular basis or as needed; updates staff regarding policy changes in specialized areas. Develops and utilizes department policy statements, scope of service, quality indicators and updates; revises as necessary. Performs staff management functions related to employee selection, on-boarding/orientation, coaching/development, staffing, work assignments, performance management and goal alignment as guided by hospital policy and procedures and collective bargaining unit agreement (when applicable). Participates, collaborates and actively engages in meetings to achieve goals and mutually beneficial outcomes. Conducts regular staff meetings to disseminate hospital and/or department/division information and allows for discussion of staff concerns needing clarification or resolution. Provides a customer service oriented culture within his/her area which includes service orientation to work processes, staff customer service and actively participates and supports development of key customer service outcomes. Actively participates in performance improvement initiatives and other activities which support unit function and overall hospital philosophy and goals within the framework of the Mission, Vision and Values of the Hospital. Competently develops the vision and structure to implement and maintain a wide care management system including organizational structure, roles, accountability in conjunction with leadership from Crouse Medical Practice. Provides direction and oversight for all care management functions across the system within span of influence. Ensures standardized processes, procedures, roles, tools and transitions of care mechanisms. Provides single source for training to support integration and consistency of process and improved transitions across the care continuum. Establishes information systems to support integrated care delivery. This may include predictive modeling, risk stratification, information system software to support work flow and house care management data. Consistently ensures robust measurement and reporting capabilities are available to adequately evaluate impact. May be asked to represent Crouse Health in state-based and Federal health care reform initiatives. Assumes other duties as assigned by senior leadership.Director of Care Coordination Requirements:
Bachelor’s degree in a related field required. Must be currently licensed as a Registered Professional Nurse in New York State. Recent acute care Utilization Management and Case Management experience. Current certification in Case Management (CCM or ACM) and/or Health Care Quality (CPHQ) preferred. Will consider other relevant professional certifications. Master’s Degree in Nursing, or MBA preferred. Interqual Certified Instructor (ICQI) or Milliman training highly desired.Benefits Overview:
