The population health team within Mount Sinai encompasses payer contracting, population health analytics and operations, care management, wellness services, provider communications and engagement, clinical integration, and population-based quality and efficiency management and improvement. The team includes 400+ employees with clinical, contracting, finance, IT, analytics, operations, and product development expertise. Within MSHP Clinical Operations, the Clinical Integration Team drive value-based contract performance by decreasing unnecessary variance on leading clinical practices and decision making (i.e. processes), establishing and disseminating measurement and transparency of the achievement of leading clinical practices and adherence to key care milestones, assure provider engagement and alignment to support clinician workflow and assure linkages among diverse programs and resources.
**Qualifications**
**Education and Experience**
? BA or BS degree required; Master?s degree including MBA, MHA, MPH, or Master?s in Organizational Psychology preferred
? 5-7+ years relevant health care experience required
? Sophistication to prepare and co-lead meetings with senior administrative and clinical stakeholders is required
? Excellent Microsoft Office skills is required
? Outstanding project management skills is required
? Experience in translating data analysis into key operational, business and possibly clinical strategies is preferred
? Experience in quality improvement and clinical transformation is preferred but not required.

Non-Bargaining Unit, E01 - Partner MSO Services - MSH, Mount Sinai Hospital
**Responsibilities**
Mount Sinai Health Partners seeks a Director, Clinical Integration who will:
(1) Serve as a collaborator and leader in select special projects as assigned by Senior Vice President and CMO, Population Health and CIN and Senior Director, Clinical Integration. Serve as key preparer of materials and analyses for select senior stakeholder meetings throughout the Mount Sinai Health System. At times, co-lead regularly scheduled senior stakeholder meetings throughout the Mount Sinai Health System. Important to this role will be:
• Ability to get smart quickly
• Professional maturity to manage executive and system impacting projects and deliverables
• As warranted, ability to maintain discretion on sensitive executive and system projects and deliverable
(2) Drive the implementation of population health optimization processes, workflows and feedback primarily at highest priority and most challenging adult primary care and multi-specialty practices within the Mount Sinai clinically integrated network of 4000 physicians and advanced practice providers. The ideal candidate will have a honed operational, relationship and quality improvement skill set. Equally important is the ability to problem solve, continuously engage and move a practice beyond barriers and to repeatable, hardwired workflows that drive success in the following areas: quality performance, reducing unnecessary patient emergency room and hospital inpatient use, documentation and coding practices to support accurate risk adjustment, and increasing patient engagement in preventative, wellness and chronic disease management.

**Compensation Statement**
Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $140000 - $230000 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
