Your recruiter will provide the most up-to-date details during the hiring process._ **Responsibilities** Direct case management services, workflow, and resources. + Formulate and implement operational strategies and initiatives in the case management program. + Direct and evaluate all services provided, including care plan development, service level determination, and complaint management to achieve performance and quality control objectives. + Provide specialized expertise, ensure resource alignment with system-wide initiatives, and updates appropriate parties on case management strategies, initiatives, outcomes and issues. + Partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care. Collaborate with a larger multidisciplinary team to improve quality outcomes using data and documented best practice. + Responsible for resolving escalated issues arising from operations in the case management department. **Requirements** Licensure / Certification / Registration: + Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required. Education: + Bachelor's degree in nursing or diploma from an accredited school/college of nursing required. + Master's degree or combination of Bachelor's degree and experience preferred. OR 5 years of experience required. + 10 years of experience preferred. + 2 years of leadership or management experience required. + 5 years of leadership or management experience preferred. **Additional Preferences** + Minimum of 5 years of Case Management experience within an Acute Care setting + Minimum of 2 years of proven leadership / management experience, overseeing Case Management teams in an Acute Care setting + Current Case Manager certification from the American Case Management Association (ACMA), or the Commission for Case Manager Certification (CCMC) **Why Join Our Team** Ascension is a leading nonprofit Catholic health system with a culture and associate experience grounded in service, growth, care and connection.
