Responsibilities:

Select personnel to supervise activities of all service areas and consult with them regarding problems.Represent facility with government and private agencies.Keeps resident’s family and physician informed of any changes in resident behavior, medications, or conditions of health.
Modifies resident care plan, care directives, and activities of daily living in conjunction with resident changes in condition
Train, coach, and mentor a team of nurses and caregivers.
Maintain schedules and ensure proper training, disciplinary action, etc. for nurses and caregivers

Qualifications: Current and good standing RN or LPN license for the state of Illinois
Minimum of 2-3 years of long-term care and management experienceClinical management experience preferred
Possess strong interpersonal skills to lead employeesPoint Click Care (PCC) or other Electronic Health Record (EHR) experience
Highly motivated, energetic, and dependable Excellent communication and customer service skills Be able to work with others in a team environment effectively, efficiently, and professionally Must enjoy the many challenges of today’s long term care fieldDemonstrated knowledge and passion for serving seniors
