You'll serve as a key resource for the Healthcare Homes team and other clinical teams, ensuring smooth operations and compliance with regulations. Coordinate day-to-day tasks of the Health Home program, ensuring efficient operationsTrack client activities and prepare monthly reports for the Department of Mental HealthProvide clerical support and assistance to the Healthcare Homes Director and teamMaintain user accounts and monitor agency usage as a Cyber Access Practice AdministratorOffer technical support for Care Manager and Cyber Access systemsEnsure timely completion and submission of required client screenings and reporting measuresAttend state-required meetings and trainings to stay current with program requirementsCollaborate with the Quality Assurance team to maintain compliance with established proceduresAssist in distributing reports to treatment teams and community providers as neededEducation and/or Experience Qualifications:
High school diploma or GED requiredAdditional Qualifications:
Proficiency in Microsoft Office and Microsoft AccessExcellent communication and organizational skillsAbility to implement innovative programs and adapt to changing requirementsMust complete New Hire Orientation and all required training, including Relias, at the beginning of employment and annually thereafterValid driver's license, acceptable driving record, and current auto insurance requiredMust successfully pass a background check, including criminal record and abuse/neglect screeningPhysical ability to sit and stand for extended periods (2-4 hours), occasionally lift up to 50 pounds, and perform other essential job functionsKeywords: Healthcare, Facilitator, Health Home, Care Coordination, Medical Records, Healthcare Administration, Patient Care, Cyber Access, Care Manager, Healthcare Compliance
