Cystic Fibrosis Center Coordinator is a key member of the Cystic Fibrosis team with oversight and input in in the Cystic Fibrosis Center and coordination of the of the team Oversees the daily operations of the Cystic Fibrosis Clinic ensuring alignment with both Hartford HealthCare and Cystic Fibrosis Foundation CFF mission vision and values Develop and implement program object and provide clinical coordination to develop and implement program objects for cystic fibrosis patients in both the inpatient and out outpatient setting to ensure continuity of care for patients and physicians Functions as contactconnection to CFF facilitating requirement of yearly preparation of deliverables for the CFF grant including financials and program accreditationJob ResponsibilitiesmiddotFunctions as a liaison between CFF and CF clinic Facilitates requirements of yearly preparation of deliverables required for the CFF grant Collaborates with CF team on the expected criteria for maintaining the expected requirements for program accreditation Collaborates with leadership team in preparation of yearly budget in addition to monitoring and maintaining budget setmiddot Serves as liaison to health care teams and care issues and care plans by facilitates the flow of information from collaborative management teams healthcare teams and committees to members of self directed Bridges the communication gap between inpatient and outpatient CF care other Specialist involved in CF care and interaction with patient and their families or CF CentersmiddotParticipates in team meetings and inpatient rounds if needed tracks and maintain patient records identifies problems prior to clinic visits and communicates with appropriate team members provides coordination between services and settings implements program objectives and provide patient educationmiddot Serves as liaison to health care teams and all patient care issues and care plans facilitates the flow of information from collaborative management teams healthcare teams and committees to members of self directed work teamsmiddotSupervises day to day activities of assigned staff to achieve performance goals in support of the operating plan Assigns reviews and modifies assigned work tasks Assist with annual performance evaluation process and personnel action as neededmiddotDevelops and administers competencies for new colleague orientation specialty care orientation annual colleague evaluation policies and procedures and equipment recommends changes in content based on needs assessment changes in procedures and equipment changes in regulatory standards and changes in medical practicemiddotOversees and coordinate respiratory staff students nursing staff resident clinic observation days and CF
