This individual reviews and edits the information included in the PAI for each patient to ensure accuracy in the scoring of functional status and portrays a reflection of the patient's care needs. In addition, the position may assist in the ongoing monitoring of quality metrics established within the unit. This position will have access to confidential information and requires confidentiality.Investigates co-morbidities to be included in the CMGResponsible for staff education to improve scoring with Quality IndicatorsReview facility nursing and therapy documentation to eliminate repetitionOversees/completes and ensures accuracy and the completion of the IRF-PAI prior to electronic transmissionServes as an educator to IRF-PAI users and maintain ongoing knowledge of the regulations surrounding PAI and the completion of it within the inpatient rehab settingCollaborates with physicians and the interdisciplinary team to make the most accurate assessment of the needs and strengths of patientsCoordinates the completion of the admission assessment of the PAI by the 4th day of patient's stay and the 4th day following discharge for each inpatient rehabilitation patient.Assists the CQPI in identifying and monitoring trends in patient care as identified through the PAICollaborates with all team members if the clinical team to ensure compliance to all levelsReports and assesses quality metrics for the unit on a monthly basis
Required Minimum Education: 4 year Bachelors Degree- Must be a graduate of an accredited RN/PT/OT/SLP program with current state licensure
Specialty/Major:
Required: Current BLS CPR card
Preferred: 3-5 years’ experience in an acute rehabilitation setting
