Job Summary: The Coordinator-Cardiovascular Services will be assigned to patients who require special coordination due to the complexity of their illness and treatment being offered, or a pattern of frequent utilization of the acute hospital and other services to assist patients with overcoming barriers that might hinder the patient from achieving health goals or quality of health services and is responsible to provides direct and indirect care for specific patients seeking Electrophysiology, Heart Failure, Cardiac Vascular, or Congenital Heart Disease services. Performs a variety of duties within the unlicensed assistive personnel scope of practice that includes, but is not limited to, following a physician�s care plan, maintaining frequent telephone and face-to-face contact with the patient along the continuum of care. Serves as a liaison and care coordinator working with the entire interdisciplinary team coordinating care between the Hospital diagnostic and procedural labs, the outpatient clinic, other external partners, the patient and/or the responsible parties and will be knowledgeable of the benefits and resources available to the patient, patient registration, data collection and administrative tasks. Works with on-site physician to manage a wide range of education and highly specialized concierge services specific to this population of patients. Utilizes interpersonal skills to maximize excellence in customer service, safe patient care and professionalism. Performs other duties as needed.
Education and Experience: High School Diploma or GED required. Graduate of a Medical Assistant (MA) required. Licensed Vocational Nurse (LVN) preferred. Minimum one year of clinical experience required.
Knowledge and Skills: Basic knowledge of medical terminology, Health Insurance Portability and Accountability Act (HIPAA) and patient rights required. Able to read; write legibly; speak in English (and Spanish preferred) and relate and communicate positively, effectively and professionally with others; use computer, printer and software programs necessary to the position; operate and troubleshoot equipment required for the position; perform secretarial duties as needed for position; be assertive and consistent in following policies; work calmly and respond courteously when under pressure; accept and follow directions; perform basic math functions; manage multiple assignments effectively; organize and prioritize workload; collaborate with nurse/physician in charge to problem solve; recall information with accuracy; pay close attention to detail; function within medical assistant scope of practice; distinguish colors, hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place; see adequately to read computer screens, medical records and written documents necessary to position.
Licensures and Certifications: Medical Assistant certification required. Active Licensed Vocational Nurse (LVN) certificate preferred. Basic Life Support (BLS) certification from the American Heart Association (AHA) required. Certified Medical Administrative Assistant (CMAA) certification or Certified EKG Technician (CET) certification through National Healthcareer Association (NHA) within 6 months of eligibility. Valid/current unrestricted California Drivers License required. Valid/current auto insurance required.
